If you reside within
the Consular district of the Consulate General of the
Republic of Azerbaijan in Los Angeles, which includes
Alaska, Arizona, California, Colorado, Hawaii,
Idaho, Montana, Nevada, New Mexico, Oregon, Utah, Washington
and Wyoming you may obtain documents from Azerbaijan
authorities on demand through our office.
To
obtain documents from Azerbaijan authorities you have
to provide the Consulate General with the following
documents:
- Application
for Document Request from the Republic of Azerbaijan.
- An
application in a hand-writing form requesting
obtaining a document on demand.
- A money order or cashier’s check
payable to the Consulate General of Azerbaijan in
the amount of $25.
If you apply by mail, a self addressed
stamped return envelope for certified mail or prepaid
envelope for an express delivery service must be enclosed.
